Recommended "Additional Insured" Liability Form

If It is available, the original Form CG 20 10 11 85 is the most desirable.  Several versions of this form were used in '85, 93, '01, '04, and '13, most of which removed a significant amount of protection from the additional insured and are not recommended. For example CG2010 1001 must also have CG2037 1001 attached. Version CG2010 0704 is NOT recommended but the version CG2010 0413 can be acceptable depending on the controlling state law and how carefully the contract is crafted.
